Below are brochures that describe the health plan options offered by Southeast Community Care. Please review all brochures before using the links that follow to access the Enrollment Form in PDF or to enroll online at the Centers for Medicare and Medicaid Services website. By using these links you acknowledge you have reviewed the documentation below.

Southeast Community Care is provided by Arcadian Health Plan, Inc., a Coordinated Care Plan with a Medicare Advantage contract. Anyone with Medicare Parts A and B may apply. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. You must use contracted providers and pharmacies for care. Limitations, monthly premiums, copayments and coinsurance will apply. Dual Plus Plan: The plan is available to anyone who has both Medical Assistance from the state and Medicare. Premiums, copayments, coinsurance and deductibles may vary based on income. Please contact the plan for further details.
