Southeast Community Care is easy to join. If you're eligible for Medicare Parts A and B, live in Allendale county and do not have End Stage Renal Disease (ESRD-Kidney Disease) you're eligible to join Southeast Community Care.

 

You cannot enroll in this plan if your current or former employer helps pay for your drugs.

 

As a member of Southeast Community Care (our Medicare Advantage Program formerly known as Medicare+Choice), you have made a wise choice for your health care needs. We have a quality panel of providers from which you have the opportunity to select your personal Primary Care Physician. In addition, there is a reduction in paperwork as well as support for your inquiries with service representatives available at our local Kennewick office with local or toll-free phone numbers. Members may enroll in a plan only during specific times of the year. Please contact Member Service at 1-800-573-8597, TTY/TDD 1-866-573-8591 Monday – Sunday, 8 A.M. – 8 P.M., except holidays. You cannot enroll in this plan if your current or former employer helps pay for your drugs. Southeast Community Care’s Medicare Prescription Drug Benefit is only available to members of the MA-PD Plan. If you are already enrolled in a MA-PD Plan, you must receive the Medicare Prescription Drug Benefit through that Plan. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third-party. Medicare beneficiaries may enroll in Southeast Community Care through the Centers for Medicare and Medicaid Services Online Enrollment Center, located at www.medicare.gov. Beneficiaries interested in qualifying for extra help with Medicare Prescription Drug Plan costs should call: 1-800-MEDICARE (1-800-633-4227) (TTY/TDD users should call 1-877-486-2048, 24 hours a day/7days a week), or your state Medicaid office, or the Social Security Administration at 1-800-772-1213 between 7 A.M. and 7 P.M., Monday through Friday (TTY/TDD users should call 1-800-325-0778).

 

If you obtain routine care from out-of-plan providers neither Medicare nor Southeast Community Care will be responsible for the costs. Members must use network pharmacies to access their prescription drug benefits, except under non-routine circumstances when they cannot reasonably use network pharmacies. You will be required to pay the difference between what we would pay for a prescription filled at an in-network pharmacy and what the out-of-network pharmacy charged for your prescriptions.

 

Southeast Community Care is provided by Arcadian Health Plan, Inc., a Coordinated Care Plan with Medicare Advantage contract. Southeast Community Care’s contract with CMS is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed.
